By enrolling the athlete, I acknowledge and agree to the following:

Assumption of risk. Football and athletic training involve physical exertion and carry an inherent risk of injury, ranging from minor strains to, in rare cases, serious injury. I understand these risks and voluntarily accept them on behalf of the athlete.

Release of liability. In consideration of participation, I release and hold harmless Joga Academy, its coaches, staff, and partners from any claim, liability, or damage arising from injury or loss sustained during participation, except where caused by gross negligence or wilful misconduct.

Fitness and disclosure. I confirm the athlete is in good health and fit to take part, and that I have disclosed any medical condition, injury, or limitation relevant to their participation.

Emergency care. If the athlete requires urgent medical attention and I cannot be reached, I authorise Joga Academy to arrange appropriate care, and I accept responsibility for any resulting costs.

Parent or guardian. Where the athlete is under 18, I confirm I am their parent or legal guardian and agree to these terms on their behalf.

Cancellation: sessions can be rescheduled or cancelled with at least 24 hours' notice; later than that, the session may be forfeited.
